Save the manuals, indeed. Tried a little too hard once. Courtesy of my 1989 325i :P

Is that like newer BMWs (well, e46 & 39) where you have to go engage reverse from 1st?

I'm not entirely sure what you mean by that, but you don't have to be in 1st to get to reverse. It's a pretty common layout for German manuals to have, reverse on the left.

Right, except for a Focus ST all my Opels, VWs, BMWs, and Renaults have Reverse high left.

What I meant was in my E46 and E39, reverse was engaged by shifting into first and shifting out of first while pushing left and down simultaneously. You couldn't go from say, second into reverse.

Most of my other cars had a ring under the knob that was lifted up, but all my BMWs were different

I take it you live in Europe? Maybe it's a European thing? I test drove an E39 540i with the six speed recently and it did not require any extra steps to get into reverse, I could just shift into it the same as any other gear. My car is the same way.

The only cars I've driven where you had to do something extra to get into reverse are my mom's New Beetle (you have to push down) and Saabs.

The E39 was euro (530d rental) the E46 was a US Spec 325i, but the rest were German. Maybe it was just those ones then. The E46 was a buddy's so I don't know the history of it maybe he jacked something up.

I had an Opel Astra wagon ADAC rental where the reverse button was in the front of the shifter (like an automatic shifter) and I had a bad habit of pushing it while trying to get in 1st gear and ending up in reverse.

Never had the experience you describe with any BMW. Sounds like some car-specific synchromesh-issue to me...

Hah, the shift knob on my '90 325i is heading the same way. It's glued together, for now. I think I'll replace it with a weighted ZHP style knob from Turner Motorsport when the time comes.

That's exactly what I did. I love the retro knobs, but the ZHP still suits the car quite well in it's simplicity, I think.
